Another place where ideas go to die is the approval process:
Imagine this one as a 1fte cartoon:
“Your idea would save this company millions, but would cost me my job. Why should I approve it?”

It’s like you’ve channeled my company’s IR&D program, but with fewer magical black boxes.
Reminds me of a suggestion that was rejected because I filed it under “Category A / Sub Cat B” instead of “Category B / Sub Cat A”.
